IUI/SO-IUI: Anyone?

Hi everyone, can I check the whole process of SO-IUI? How many days are we supposed to consult the doctor for scannings etc before the actual procedure? I need to plan accordingly as I might be unable to get leave during this period and I am no longer WFH from 2 June onwards.

it depends on how closely monitored you are by ur dr.
i have done it with 2 dr before, 1st need to visit day 2 to day 5 while having menses, then collect medication/injection, then another appt is day 13 to see if your follicles are ready for IUI. If all is good, Trigger shot at clinic, Day 14 have to go back again for IUI.

2nd dr monitor me on day 2 of my period, collect medication ,follow by check up on day 10 & dr ask me to trigger myself at home on day 12 night , day 14 IUI.

Poly will refer you to government hospital. When you wanna start IUI you need do the checks for the doctor to see if there is any underlying issues. For me I do the checks first before heading to IUI

I wanted to go straight with IVF but doctor suggested that I try SO-IUI first and because of the CB, I was hold back with the procedure and now thinking that I shld go straight with IVF. Do I need to make appointment again or can I just call KKIV and change my procedure to IVF? Or you think I shld give SO-IUI a try? Im so lost right now.

I went to a private gynae previously to check out why i had irregular menses.. she only did a ultrasound and diagnosed pcos from my menses history. Was on metformin but only for a few months.. After that tried clomid but got horrid side-effects. Gave my wallet a rest for a while and then now deciding to go for IUI. Going to ask for a proper check first though to make sure that everything else is fine.

Anyone did IUI with Dr Yeong Cheng Toh from Tow Yung Clinic before? Any successful stories?

I had 2 normal IUI cycle with Dr Lee Keen Whye. 2nd one ended up in a MC at Week 7. I decided to change doc and just did my 3rd SO-IUI with Dr Yeong today.

For those who are keen to know about the cost involved, hope below helps!

1st IUI (Not SO) including scans - $994.50
2nd IUI (Only with 5 days Clomid) including scans - $694.50

Perhaps you should discuss with your husband too. I did So-IUI and to not able to yield any pregnancy back then made me feel like all these jabs were for nothing. It could have been done for IVF. I chose So-IuI becox I didn't want it to be as invasive as embryo retrieval. Ended up, I had to do IVF still.
That is how I feel. Nevertheless, I hope your SO-IUI is a sucess!
 
Hi wonderful ladies,

This month im tracking my ovulation w clearblue 2 days peak ovulation kit. The thing is, ive had static smiley since 30/6 till 3/7. Im not sure if ita faulty or what, but what does this mean? Its excited whwn i saw this static smiley but when i keep seeing it for like few days in a row, its kinda makin me confuse. Anyone experience same thing?

hello @farvind is this your first SO-IUI? good luck to you and hope this will be your month!! it's good that you don't test and just wait for blood test..i tested because i was too impatient haha.

My case is also an unexplained fertility case, my PCOS has “disappeared” and I lead a pretty healthy lifestyle. We have decided to move on to IVF after doing SO-IUI just once as there are too many uncertain points as compared to IVF. Moreover, the injections are all gonna be very similar... People say that IVF is more invasive but when you are going to give birth, it’s equally invasive too or worse.

PS: I noticed that the medication I took for SO-IUI kinda screwed up/extend cycle/extend luteal phase.

@Zerelda hey thanks for sharing! i too wanted to do ivf right away instead of going through iui but maybe because i am going through a subsidized route, KKH strongly recommended me for two cycles of SO-IUI first just to see if i could handle it..u were on a regular cycle and it delayed your period?

I did SO-IUI in Singapore last year but failed so I am going onto IVF directly - gonna start soon in the United States (as I am based there). My doctors/specialist recommends IVF cuz of my husband’s age (41). I am 32 this year and they said starting IVF earlier will have better chances. I would say that if you have the budget - go for IVF. The process isn’t going to differ much as you will still be getting almost all the same injections (only an additional one on releasing multiple eggs). Other optional components include assisted hatching, pre-genetic screening on embryos and injecting sperm directly into egg for optimal fertilization.

PS: I was diagnosed PCOS years ago but made many lifestyle changes and my ovaries no longer appear PCOS but the tag doesn’t get removed as PCOS is for life.

I decided to go for ivf as we have a few friends who did SO-IUI 2-3 times and still failed - success rates for SO-IUI still isn’t as high... I feel that most doctors in Singapore cannot give a strong recommendation.. To them, it seems like cuz SO-IUI is easier, they ask you to go for it first. I honestly think it differs from person to person. To me, unless both of you are super young and have zero issues - then to go for SO-IUI... The worst thing to happen would be to conceive via SO-IUI but have a miscarriage occur. IVF allows pre genetic testing which prevents such miscarriage from occurring. Moreover, you usually only suffer the medication once - cuz you will be retrieving multiple embryos and probably freezing them. 1 SO-IUI actually screwed up all my hormones badly that it took me months to recover and allowing my hormones to stabilize.

Well my hubby is older... His morphology level is also slightly lower than average (he got a 3 and 4 or more is better) but the doc says it’s ok, nothing to worry about. I wanted to push for ivf then but I came back to Singapore on the 5th day of my cycle so we just went for SO-IUI to see if we hit the jackpot.. but unfortunately didn’t. After our SO-IUI failed, we realized a lot of our friends tried 2-3 rounds and failed and eventually went for IVF and wasted their subsidies.

Yes there are miscarriages associated with IVF but way much lesser if both husband and wife are considered to be generally ok. IUI has a lot more...

Yes, I will be starting IVF process - still don’t know if it’s the short or long protocol as the bloodwork results will be required first (I think Singapore doesn’t have that, all short). Depending on your hormonal levels, you will have a customized protocol. We had to put on hold due to covid but our area is way much better now and the hospital facilities for our area is also one of the best in the US... I am not sure if the treatment process is better/different but it will be almost $0 for me (just pay $40 every time I see the doctor) as my company’s insurance covers 6 cycles of IVF/IUI + prenatal care + birth + hospitalization + postnatal and infant care.

Short Protocol + Dual Trigger (prevents OHSS)
Day 1 to 10: FSH + LH injections
Day 5 to 10: GnRH to prevent pre-mature ovulation
Day 10 to 11: Lupron
Day 12: Trigger + Egg Retrieval (can select intracytoplasmic = inject sperm directly into egg; basically find the best sperm and inject directly)
Day 13 onwards: Progesterone (if pregnant will continue).. if not, will stop and wait for menses.
Day 15: pre-genetic tests on embryos for like missing chromosomes, genetical issues.. all these embryos will be discarded.. only the best embryos will be allowed to grow to blastocyst stage
Day 17: Blastocyst stage embryo transfer. Can select assisted hatching for higher chance of implementation
Day 26: Pregnancy test


Long Protocol (usually used for PCOS patients)
Day 1-14: Birth Control Pill
Doctor explained that PCOS patients tend to release a lot of eggs but majority of the eggs are very low quality therefore causing miscarriage even if they were fertilized.. many turn out as grade B embryos (what you want is A grade). Based on my last scan in Singapore with gynae #3, I do not have PCOS (only 5 follicles with Letrozole). However, previous SG doctor (gynae #1 and #2 said I’ve PCOS but no scans to show).
Next 10-12 days - Lupron to prevent pre mature ovulation + stimulation injections
Day 13 - hcg trigger + progesterone all the way unless didn’t get pregnant
Day 15 - Egg retrieval
Day 17 - pre genetic testing
Day 19 - embryo transfer
Day 28 - pregnancy test

Even as a subsidized patient, I am preparing to fork out a few thousands for each IVF cycle so it IS a financial concern for me and one that I hope I do not have to embark on multiple times to find success..

That’s probably the only good thing about the US health medical insurance. Definitely more comprehensive than Singapore.. I’m also lucky cuz my employer gave us a good policy.

There is 1 thing I note about Singapore though, they follow a general scale when measuring hormone levels and usually only have 1 method (although different types of medication). I’ve read too many cases of OHSS in Singapore. I feel that every body is unique and has its own set of hormonal issues (like you might need more/less estrogen, more/less FSH/LH) therefore there should be customized protocols and not a 1 size fits all. My first 2 gynaes in Sg didn’t make me feel very confident - the way they analyzed my results.. did not explain much and simply determine PCOS. My last gynae explained and was pretty good but still not as detailed as what I get in the US. Maybe you will need to ask your doctor more questions if you want things answered.

But again, everything in the US is a business, including education and business. Therefore, success rate and customer satisfaction are very important to them. A low success rate or customer satisfaction will make the center look bad and lose business.. therefore they will do whatever they can to maximize success.

I was seeing Dr Loh from Thomson and almost 10 months later - the conclusion is physically there are no problems with neither me nor my husband, yet there is still no baby. Dr Loh also keeps asking us to try naturally and isn't discussing the option to IUI.

At this point I have already made up my mind that I am going to try IUI.
I am thinking of either heading for a 2nd consult with KKH/ NUH (with a referral from Poly) or just go back to Dr Loh and look into IUI.

The concerns I have on hand:

Time - I am 35 next year and I am worried taking the KKH/ NUH route might be long as it is not private.
Finances - How expensive can it really go if I chose to go Private? How much could the total be if I opted for Govt.

Not all will get fertilized. What is hubby’s motile sperm count like and the morphology?
Ok tts a relief. Was worried cos doctor asked me if i'm ok if it results in multiples. I personally think the chances are really low rite. Can't remember exact but count is good and morphology 3%.

No all varying sizes 16,14.5 and 13 mm mostly. Ur egg size 13.5mm isn't that considered a good size?

I had 5. 1 was at 15.. the other 4 were like 10-14. My IUI still failed but very likely due to husband factor. His morphology was 3% and motile sperm was 5+ million.

They require another sperm analysis to confirm that my husband’s motile sperms are indeed below average. I’m only gonna see my doctor on 8 sep and will move from there.

Not really.. i understand follicle size should be abt 18mm and above to for fertilization but of coz we wouldnt be able to monitor it 24/7 so the egg could have grown in between the scans and by our ovulation/procedure..

Yeah.. they already increased my dosage for puregon compared to the first cycle coz my body wasn't too responsive, but this cycle feels worse lol. Hopefully your treatment will all move quickly and smoothly from 8 Sep! I also booked an appt to proceed with IVF in the third week of Sep should my so-iui cycle fail the 2nd time.. good luck to us haha..

Just want to share my IUI journey so far.

Background: TTC for 18 months. Jan 20 went to a normal gynae for fertility check and was on 6 months clomid. No results.

21 Aug (Day2): My 1st consultation with Dr Loh SF from Thomson Fertility Centre. He looked through all my fertility check results and asked me to do IVF directly. I wanted to try IUI first so he agreed and said I can do once. If fail, then IVF. Hubby has low sperm count. Don't have any issues on my side. I kickstarted the process in the same day since I was on Day 2.

Was prescribed 5 days of Letrozole, 5 days of Puregon and supposed to see Dr Loh on 28 Aug (Day 9) for ultrasound.

28 Aug (Day9): Womb lining was great, I have 2 follicles, 1 at 13.5mm 1 at 15mm. Dr said I need 2 more days of injection (1day Puregon 1day Menopur) and ill be good to go for the trigger shot. He scheduled my trigger shot on 30 Aug (Day 11) at the 24 hour clinic at Thomson Medical and the actual IUI procedure on 1 Sep (Day 13)

1 Sep (Day 13): Procedure took 5mins and it wasn't painful just uncomfortable. Sperm count was extremely low at 700k. Hoping for the best but managing my expectations and prepping myself for IVF next!!! Was also given 14 days of progesterone inserts (via vagina).

16 Sept (Day 28): Beta blood test

Just want to update you guys that my first SO IUI failed. My spotting came on day 27, e day before my BT. Wanted to start on IVF immediately on Day 2 but doctor said no go due to functional cyst. Now I'm taking the time to strengthen hubs and my body, hoping to start IVF next cycle. Happy for any tips and advice for the next 1 mth!

I have cleared all my tests and am starting on IVF. Just wanted to share my plan in case any of you are curious and planning to start on IVF. My doctors have put me on the Lupron-Gonal F Multidose Antagonist Protocol. This is a protocol used usually for ladies with irregular periods and/or PCOS. My current age profile is 32Y 4M and I have irregular periods but not considered PCOS; husband 41Y 1M has fluctuating motility (2.8mil to 5.7mil).

We tried IUI once last year in Singapore and decided to go for IVF after we failed. IVF was delayed due to COVID.

PS: I am currently seeking treatment in the United States and therefore my medication/protocol might be different. This is only FYI in case any of you are considering IVF.

Prenatal medication is a must! My US doctor made me start on prenatal the moment I said wanted to do IVF.

We do liquid vitamins from MaryRuth Organics and Nordic Naturals.

Me: Prenatal Liquid, Prebiotics and D3 from MR + Omega from NN.

Him: Essentials+ + D3 + B Complex + Prebiotics from MR.

PS: Cuz weather is getting cold now. We also take half dose of MR vitamin C before sleep.

Eat clean, take lots of protein (eggs), drink lots of water and don't forget to go out and have fun (relaxes you). Don't give up hope:)
 
Hi Moon08, I just did my first SO IUI at Kkh, was referred by polyclinic and doctor is Dr Chua Ka Hee. Did the usual blood tests (i.e whether if I'm ovaluating etc...), SA followed by HSG and was given all clear before we proceed to SO IUI. He gave us 3 options, continue to try natural way since all results were good, iui or SO IUI but due to age we decided to go with SO IUI.

Started first cycle early December, had to jab myself with Puregon to help follicles grow, went back KKH 3 to 4 times for scan to check on follicles and had to do additional shots to stop ovulation. And finally on the last 3 days before the procedure itself trigger shot and then the procedure.

It's correct that the procedure will not be administered by the Dr we are with but it was one of the doctors whom we saw during our follow-ups post scans. Overall process took less than 10 mins and I rested for around 30 mins and went home.

Now in my TTW have to go back for blood test 30 Dec.

The nurses on site are all really nice and friendly so need not worry!

They only gave mc cuz the whole procedure was not invasive and not much need for bed rest.

Our iui failed though, now in ivf which is alot more invasive from jabs to eggs retrieval so they gave HL.
