Is Deed of Separation necessary to divorce based on separation ground?

I am keen to know if the above is necessary? And can the Deed of Separation be prepared by ourselves and witnessed by relatives? Instead of spending few hundreds dollars to get it prepared by the law firm? Appreciate if anyone is willing to share the Deed of Separation with me. Thank you
