IUI/SO-IUI: Anyone?

Morning all, Im on Day 1 of my menses. Tmr going for scan and start jabs again. 2nd try on So-iui. Since i know i react well to the medications, now is just all about the right timing and DH troops to be strong and healthy. I really hope i can go through with the procedure without any complications. For those of you who are not aware of my situation, the first time i started SO-IUI, they gave me clomid and jabs which resulted to 6 big eggs and had to cancel the procedure. This time round, they are giving me jabs only. I hope this mth will be my lucky mth and to all of you who are ttc.
 


Jen: Metformin is used before clomid if the doctor suspects that the PCOS is caused by insulin resistance. These cases are usually those with BMI >25. Maybe US has has more obesity caused PCOS. Most docs here start with clomid followed by metformin+clomid.
 
Hi ladies, can I ask...

1. Anyone proceeded with IUI with less than 1 million motile sperm? Did you succeed?

2. Anyone did IUI when follicle is fully ripened (in my case 25 mm) but not yet released?

Thanks. Doc suggested postponing my IUI due to low number of motile sperm.

Peggy: Glad to hear your follicle has grown so big!! 25mm!!

for the motile sperm, I can't comment.. But, how do you know the no? The one I got is total count, % of motile and % of morphology..
 
Hi Gals,

Can i know which Physician or Acupuncturist is good? Dr Zou or Dr Tan?

The hospital just called me. My 3rd cycle of SO-IUI has failed. really very disappointed.

Should i carry on with IUI or go straight to IVF. Should i try seeing a chinese physician?
 
Saralyn, the gynae just told us. He didnt give us any report though.

PSP, take it easy okay... ~hugs~ Jia you! There's still hope. Dont worry. I did one cycle of acupuncture before and it helped me shorten my cycle and bbt was good with definitive two phases. Cant really comment on physician.
 
Peggy, which doctor you with? the doc should know ur DH's sperm quality before start IUI process and now ask you to abondon IUI procedure? which CD are you now? since already do so many injection, although motility low but you just need one to strike. Ur egg size is good! go go go with it.
 
psp, dont be discouraged. although our journey is long but definitely there is one day we will reach our destination.

i seen Dr.Zou for 3 months, she helped me to improve by BBT by a lot and stable it. but i didnt go back to her for 3 months already. i gave myself a break as the medication, acupuncture, BBT really makes me stress. personally i like Dr.Zou so much, she is so motherly and so caring.
 
Peggy: I suppose you DH did the sperm washing and your gynae got the report after washing ? If I were you, I would consider:

1) Cost of medication already taken vs cost of IUI. If the cost of medication is much more than the cost of IUI, +1 for going ahead with IUI
2) Time factor - if time is a crucial factor in your ttc planning, +1 for going ahead with IUI.

Regarding the egg release, if you are doing IUI, I suppose the gynae will give you the hcg trigger shot ? IUI is usually performed between 24 - 48 hours of trigger shot and it is assumed that the egg is released xx hours after the shot.

Hope my 2 cents worth will help you in your decision.

I agree with Ganbatte, talk about hcg trigger shot. i dont recall i have this shot after my first SO-IUI. Dr.Zou told me about this also but i really dont have this.

Is this hcg trigger shot critical for SO-IUI? meaning it will increase the chance of pregnancy?
 
Thanks Ganbatte, that was what my previous gynae told me, I am normal weight (BMI 20) and so no need Metformin. This seems to be the main school of thought amongst gynaes in Singapore.

But when I started seeing Dr Charles Lim he said from his 20+ years experience with PCO patients the majority respond well to Metformin, even thin girls.

I started researching more online and also bought two books on PCO. It was then I realised that there is another school of thought which believes insulin dysfunction is the root problem for ALL PCO patients, and Met helps to correct this, and in fact is especially effective for so-called "thin PCOS" cases.

Prosepina: HCG trigger shot helps the gynae to pinpoint the egg release timing. TCM uses BBT to estimate this timing. They have the same objective, which is to pinpoint when ovulation occurs. Taking BBT requires a lot of discipline and has greater margin of error. Thus, gynae in general, does not recommend this method, unless you are doing ttc via BD.
 
Jen: Yup, I must agree that most gynaes in Singapore have a 'standard' set of procedures which they will recommend. My BMI was also < 25 too, but was also prescribed with metformin+clomid after clomid fails to react. From my gynae's experience, he had patients who responded well to metformin+clomid. Its really a 'trial-on-error' process. If it doesn't work, they will recomend you move on to the next step. The key for me was to persist!

Prosepina, I am with Dr C h e w (NUH) under subsidy. I had no injection though. Just natural with clomid. But I had late ovulation (i think). Today is CD23 with 25mm follicle and based on today's scan, egg has not been released.

Ganbatte, since I had no injection, cost already taken is minimal. DH's sperm after wash have less than 500,000 motile. Very sad... anyway... I had HCG injection on Sunday, 1/4/12 at 8 pm. Planned IUI was today, 3/4/12 at 2 pm. That is 42 hours after HCG and follicle has not burst!!! Im thinking, will this follicle burst? I will probably just BD tonight and tomorrow night. In case it burst.
 
Hi Peggy, u have waited 1 month for this cycle. Like what my husband say give it a shot. Anticipation and guessing won't give us an answer so why not give it a shot? Who knows might be lucky? Try then give ur hubby supplements to improve his quality? Or ask doc for some supplements. With his test results doc can give supplements. Most of the time, the sperm does not have well formed head, tail or body. Dpnt give up! Best of luck!
 
Peggy: It is rare for a matured follicle not to release the egg. There is still a good chance that the egg will be released and you only need 1 good sperm to get it fertilized! If you are doing BBT, you can use it as a tool to gauge the ovulation. Don't give up hope yet !

psp: I did not try acupuncture with Dr Zou. I went to see her afew months to take those chinese herbs, she did not ask me to do acupuncture.. However, I saw alot of ladies doing acupuncture.. She is very nice.. very motherly.. But remember to call for appointment. Sometime even with appointment, I got to wait for 1 hour to see her..

How are all of u doing? Have been MIA for a while as i was in hospital past 5 days.. I've lost my bb.. My bb was a fighter n fought hard.. Had no choice but to go thru d n c yesterday at 8 wks 5 days.. It was painful but my hubby n i will be strong.. I told myself there are so many of u trying so hard here, how can i give up..

I'm very thankful for all the doctors at NUH that never gave up and gave me hope n courage to fight, until the pregnancy tissue starting coming out from the entrance of womb did we eventually accept the baby needs to go nw..

I'm not here to discourage any of you.. I want to say dat i will try hard again like all of you here.. I'm sure one day we'll all have a healthy baby dat we'll love and cherish very much..
 
min81: So sorry to hear that.. Pls rest well and take good care.. I heard you need to do a mini confinement even in such situation.. So, remember to eat well and look after your body.. Let us come together and encourage each other over here while we try for one!!!

Ya my mum can cook confinement food so i stay at my parents' in the day.. Had not wanted my family to know but my hubby had to inform them as i was sent to hospital in emergency situation..

I will rest for a bit, go back to wk 2-3 mths n quit my job when starting to try again.. I hope by then i can hear success stories here too..

Peggy: I think a lot of miscarriages have unexplained causes.. I read up online.. They say sometimes it could be chromosomal abnormalities.. Maybe dat's y mine was growing so slowly with a faint heartbeat..

i failed again this time... not sure whether to continue the next cycle. gynae said to check my fallopian tube and go for IUI the next try. i'm not sure whether to go for the check because i need to go for a surgery. discouraged... sighzz

How come need to go surgery? Checking the tubes no need surgery. They will just scanning and put a dice in to see if it's flowing out.

Checking tubes dont need to go for surgery as what dragon mentioned.. Unless you are going for another surgery first before checkin your tube?

Is this your first iui? I would suggest to go for check for the tubes. In the event they are blocked, no point you do iui..

bbbb's failure.

I am joining too... Saralyn, last night I didnt BD. Maybe we were too tired.... maybe we were too shocked that the motile sperm count was too low. Maybe it was a blow for dh. dh could not perform...
Personally, I feel scared.... i dowan our obsession in ttc to affect my marriage and relationship with dh.
It is how I live now with dh that matters. Not how we will live when we have a baby. I keep looking forward to the future that might not even exist. In the mean time, am I going to live on a roller coaster of hope, fear and sadness every month? I am on a verge of giving up on ttc... especially when my relationship with dh is at stake.
 
the gynae said i have to go for GA so it is a surgery... she said i will not be able to go for the easier method... it will be too painful for me. i am quite scared and worried now.

the surgery is quite expensive too.

the gynae advised since we are not successful with AIH, we can try for IUI but it is better to go for the surgery to check if there is blockage in my tubes. if it is blocked, there is no point going through the process.

is ok..i got a best friend. She just got to know her hubby has no sperm count at all. (even worst than yours) but she is telling me, is ok not be a mother, is not the end of the world. We can only accept things that we can't change and change whatever we can.

ask ourselves, why do we need a child? i want a child just to complete my family. If by then my dh no longer love me, then is that family complete?

Don't be too depressed k.. Give yourself time to get over a failed cycle.. It's not easy.. It's very important for u and ur hubby now to be together, to express your emotions to each other, to discuss how you two wish to continue your ttc journey, to be open.. Give your hubby time too.. I believe it's not easy for him too..

We hope to see things work out for you k.. Pray for strength..

Sperm replenish new ones every 3 mths. Ivf helps too. I've read some gals their dh low sperm count and they managed to get preg naturally and some went ivf too.

Discuss with your hubby about how he feels. Assure him that it won't makes you love him lesser.
 
Prosepina, ~hug back~ I used to say to dh that my life will be c o m p l e t e with a baby. But he told me that, with me, his life is c o m p l e t e. Sigh... I think I have to change my mindset.

Min81, thanks! I believe that what I am going through is nothing compared to what you have gone through... and yet you can still be strong and give encouragement to others. Really appreciate it.

Dragonchew, I think dh is not ready for even another IUI, not to mention IVF. It's just that dh has been seeing Dr TanKS for tcm and acupuncture, but the sperm count is deteriorating instead of improving. This journey is really emotionally tiring.
 
bbbb: I suppose your gynae is recommending laparoscopy? Do you experience any pelvic pain or symptoms of endometriosis ? If your gynae suspects that you are suffering from endo, he/she will probably recommend laparoscopy. For checking the tubes, usually HSG is performed.

Peggy: ttc is definitely a physically/emotionally/financially draining process for every couple. When you hit the wall, take some time off, reward yourself for efforts made so far. Every effort made, no matter how small, is a stepping stone towards your goal. When you have accumulated sufficient energy, redraw your plans and press on! For my case, DH and I did a lot of communication between each failed cycle to plan out the possible paths for the next cycle. In the process, we will encourage each other to keep looking ahead, because our goals are ahead of us, and not behind.

Peggy: *hug* don't be so upset... Everytime when I see red I break down but I will tell myself new cycle new hope again.. Give yourself sometime to feel upset, but not too long.. quickly bounced back and be positive.. Ttc is never an easy journey for us.. Nevertheless, is so sweet that your dh says such thing... Give the two of you some break.. And try again.. You never know... All we need is just one egg and one sperm.. Our time will come.. Just a matter of when..

Bbbb: I went for lap before.. Cost wise it really depends on the doc... I have a few friends who went through and all the cost varies so much.. Anyway, if you having some symptoms of endo is best to go for this surgery.. Don't be afraid..

One food which a tcm told me not to let my dh eat is Snow Pea. Maybe is it his diet that is affecting the sperm?

Maybe you take a break from IUI and try naturally? Just relax and enjoy bd. I know it's easier said than done. Don't give up yet.

I'm worried and scared. I haven't decided whether to go for it.

My hubby has gone for the sperm test. The gynae said the mobility is not ok.

Peggy: dont be too upset. I understand how you feel. In fact, all of us has gone thru these difficult times. Our motive is just to have our own baby that why we have to go thru so many pain. We just have to keep trying and NEVER give up.
 
Bbbb: hm.. Maybe your gynea wants to be sure that you are "healthy" inside... When you go for lap, they will check for your tubes at the same time.. Maybe you can suggest to do hsg to your gynea first instead of straightaway to do lap.

Don't be dishearten, ya. Have faith and don't ever give up. I am sure your dream will come true.

Just to share with you that this tough ttc journey has in fact made my bond stronger with my hubby. Same problem as you. My hubby's troop motility and morphology also quite bad. I know he is sad even though he don't show it. I talk to him, let him know that is ok and its beyond our control. We just have to do our best; go for tcm, he to take vitamins that gynae prescribe and stay healhty. I guess being positive, understanding and caring, he open up and agree to go for IVF after our failed first IUI. We decided on IVF as it gives us much higher success rate. Somehow, I can sense that my hubby love me even more. I am starting my Lucrin jab this Monday and I am excited about it.

So, don't worry. Have a good chat with your hubby. Or take a break and go for a vacation. And come back stronger and try again.

You can consider IVF. Many has succeeded via IVF for similar situation like ours.

Don't give up, seven years ago the mention of artificial insemination is reason for husband and I to quarrel. Now he is the one who urged us to go for it. He also has low count and not good quality. Doc gave supplements he was also half hearted in taking. After he knows his sa results, he then took what gave and the vitamins I got regularly. Well I guess the journey is tough but so far he is supportive and it made us closer too. Give ourselves a break and come back on the journey when u r ready. Sometimes in our anxiety to get a baby, we strained the relationship without realizing it. Easy to break but hard to mend. Go on a vacation and rediscover each other. Take care and hugs
 

bbbb: The diagnosis for infertility usually has a chain of orders for the procedures to be performed. Laparoscopy is lower down the chain compared to HSG. My gynae recommended HSG before we did our IUI as it was the 'standard'. After discussing with DH, we decided to go ahead with IUI without HSG to save some time. The decision is yours, but you ought to get the facts right before making the decision. The SA results will also affect your decision, so, take a good look at the figures on the report.
