How is the CDA opened?

You may choose to open your child's CDA at either Oversea-Chinese Banking Corporation Limited (OCBC Bank) or Standard Chartered Bank (Singapore) Limited. As a parent, you can nominate yourself, your spouse or another person to be the trustee of our child's CDA. The trustee must be at least 21 years of age and is not a bankrupt.

To open a CDA at the bank, you need to complete the relevant CDA application form. The forms are found in the Baby Bonus kit available at the hospitals (birth registration counters). For an eligible second, third or fourth child, OCBC Bank or Standard Chartered Bank (Singapore) will open the CDA automatically for the child if you, the parent, are a Singapore citizen or Permanent Resident, are nominated to receive the cash gift and manage the CDA and submit the CDA application form at the birth registration counter (together with completed Form BB) .

You should submit the following documents at the birth registration counter:
a) Completed Form BB attached with a copy of your bank book (page
with account number and name)/latest bank statement; and
b) Completed CDA application form of either OCBC Bank or Standard Chartered Bank (Singapore) attached with copies of your NRIC, your child's birth certificate and
your bank book (page with account name and number)/latest bank statement.

Subsequently, the bank will send you (Trustee) the CDA number, Baby Bonus Card and PIN and Internet and Phone Banking PINs (if you had applied for these). One Baby Bonus Card will be issued for each CDA.

If you had submitted Form BB at the birth registration counter, but did not submit the CDA application form, MSF will send you an authorisation letter to open the CDA at any OCBC Bank or Standard Chartered branch. You (Trustee) must produce the authorisation letter (original), a copy of your child's birth certificate or certificate of Singapore citizenship (whichever is applicable) and your NRIC or passport when you open the CDA.

The above FAQ provided from babybonus.gov.sg is incorrect. The bank does not send you the CDA number, bb card etc directly, one still need to visit the bank to open CDA with the authorization letter, even if one submit the bank application form at the one stop service counter.
