IVF with Dr Tan Heng Hao from KKH

v informative n helpful mumwannabe.
Hope your upcoming scan shows healthy mature follicles.
btw,any idea how long the initial briefing(inclusive of blood tests) at kkivf is?
scheduled for next week, wondering if i should ask hubby to take full day off?

Thanks jkoh! My scan is scheduled for tomorrow morning and I'll update here on the progress (if any) and the next course of action. In the meanwhile, there is nothing I can do but to send telepathic messages to my ovaries and follicles to buck up! Keep us updated after your first appointment on 6 May 2014. :)

Thanks storkeblessings, I hope so too! My initial KKIVF briefing was scheduled at 1.45pm and we were done around 5pm (including the hubby's blood tests). If your briefing is scheduled around the same time, you and hubby might want to take just the afternoon off (especially if you are hoping that hubby can attend subsequent appointments with you, so that both of you can "save" on vacation leave).

Hi all,

The scan this morning (2 May 2014) showed that the biggest follicle was grown into a big fatty (23mm) and I think it is written off already, i.e. unlikely to retrieve a good ovum from that follicle. The more heartening news is that there were 2 other bigger follicles of about 14.5mm and 12.5mm, which means the rest are indeed still growing. I'm thankful that my body is reacting (maybe those telepathic messages worked a little!) and can only pray hard that the oocyte retrieval (egg collection) which is scheduled next Tuesday (6 May 2014) would yield good results. As of now, apart from the 2 bigger follicles, there are also two 10.5mm follicles. Hoping that they will also mature into good eggs by Tuesday!

Just to share a bit more of my experience at KKIVF Centre today. (Warning: The crowd is terrible when you visit the day after a public holiday. All the staff also seemed to be less jovial, hehe.) I arrived at 8.25am and did my scan around 9.30am before seeing the doctor at 10.15am. The doctor asked me to sign a consent form for the oocyte retrieval before I was sent on my way to the pharmacy to collect a concoction of injections. After this, it is another 1.5 hours of waiting to see the nurse who will advise you on the ovum pick-up schedule as well as the "timetable" for injections.

Basically, after you are assessed by the doctor as "ready for oocyte collection", the following will happen:-

1) You will be informed on when the oocyte collection will take place and sign a consent form that informs you of the possible risks related to this procedure.
2) The doctor will also advise on the injections that you require and you are to bring the prescription(s) to the pharmacy to collect the needed drugs (this is still on Medisave for me at this point in time).
3) Return to KKIVF Centre and press Queue "4". The nurse will then brief you on the ovum schedule and guide you on the necessary injections to be taken and when to administer them. [For me, I'm to continue with the Menopur (150iu) and Puregon (150iu) for today and tomorrow. I did the Orgalutran jab this morning before going for my scan and this has to be administered daily until Sunday (4 May 2014). The last Orgalutran injection will be on Sunday at 9.50am sharp.]
4) If needed, a nurse will guide you on how to do the injections (and do them for you too!).
5) A nurse will call you to inform you when you are required to take the Pregnyl jab that will "mature" your ovums/eggs for the oocyte retrieval. This will take place the night before the procedure. You will also be required to fast from 12 midnight onwards until after the procedure, which will be carried out the next day. This means no food and water too.
6) On the day itself, you will be given an appointed time to arrive at KKIVF Centre, sans make-up, nail polish, jewellery and contact lenses. The hubby also has to produce a fresh semen sample on this day and deliver it in-person. You can get a sperm container from KKIVF Centre if he wants to produce it at home (this is what I'm doing too). But you will need to get to KKIVF Centre within an hour from production.

I will share more after the procedure on Tuesday (6 May 2014). If you have other queries, let me know!

P.S. Please wish me good luck too! ;)

Best of luck mumwannabe1984!
You are almost there...
Gonna reap the fruits of your labour in a couple of days
Hang in there...You will do great!

Thanks storkeblessings! Will update after the procedure. Remember to ask as many questions as you want at the IVF briefing! :) Only time when they can answer your questions promptly. When is your briefing this week?

Jkoh, we will be in KKH on the same day! I'm scheduled to be there by 7.30am. :D

Hi momwannabe1984..:) i felt like I was reading my own story! Born 1984, married in 2007 too! Thinking of going for IVF.. As my husband's sperm count is low! Planning to visit kkh this week for advice...
You are in my prayers!! All the very best and loads of luck to you. :) thanks for the detailed account of your experience.. It is very helpful

Hello there, 1984too! And many thanks for keeping me in your prayers and for the good luck that you are sending me way! Welcome to this forum, I'm also hoping for more to come on board and share their experience. Sometimes, it is also a form of bonding and support for those going through this process. At times, the trying to conceive part can wear us down mentally. Better that we "vent" our frustration here than on our loved ones! I've been through it when trying to conceive naturally. The poor hubby literally has to "do it" on demand. It affected the intimacy between us both for a while, but thankfully, the hubby is very very sweet and loving throughout this experience. :) By the way, which consultant are you thinking of seeing at KKH?

I had my Pregnyl jab last night (4 May 2014). I was scheduled to arrive at the 24 hour O & G Clinic at KK Hospital by 8.30pm, but hubby and I reached at 7.52pm. When the nurses see you with the cooler bag containing your drugs, they will ask you for your IVF forms and then you are required to take a seat and wait patiently for the nurse to call your name. You will then be ushered into one of the rooms (where you will see many other patients lying there - I think they are those who are there to see the doctor cos they are unwell) and a nurse will first take your blood for tests. After the blood test, another nurse will bring you to a bed (curtains to screen you from other patients) and administer the Pregnyl jab. You have a choice of being jabbed in the thigh or on the butt, but it has to be administered on a muscle. This means that it will be slightly more painful than usual.

Before I had my jab, I heard many horror stories about how painful it was. I must say that the anticipation is worse than the actual injection. Yes, the injection into the muscle causes you to flinch and clench the muscle uncontrollably, but you must try to relax. I was quite lucky in the sense that the nurse manager who administered the injection for me was very experience and the pain was minimal. You can resume normal activities immediately afterwards. I did not feel much discomfort, but do note that it may vary from patient to patient.

This morning, I did feel slightly bloated in my lower abdomen but nothing unbearable. Will update again after my oocyte retrieval tomorrow morning. It is scheduled at 7.30am! It is so early, but I guess in a way it is good. The earlier it is done, the earlier I would know the quality of my eggs. But the poor hubby will have to wake up super early to produce his necessary goods! So do remember that you are not alone in this, the poor guys have to do their part too! And it is probably not so easy for them too. :)

In the meantime, I hope our wishes for a baby will come true soon! Good luck to all of us! <3

Hi I am fr 1984 too, same prob as u, low sperm count. Natural/IUI totally doesn't work for us unless ur hb sperm count can improve (and he must be willing to try the medications, my hb not willing). The next option is ivf. Hope ur visit is gd and u get suitable advice!

Good luck to u! Dun worry about no. of follicles. I have bfp before with 2 follicles. It was same exp like u, one dominant follicle and a slightly smaller one, rest all cmi. But that time the doc chose togo ahead with ER and not wait anymore.

Thanks for sharing your plights gals.

For my case,I married late.I'd thought I will concentrate on my career and start family-planning before i reached 35.

Well.things dun usually go according to plan.Im past 35,tried for 2 years(not every cycle:either my hubby or I stressed or away on biz trips)

Had 2 failed IUIs before in Glen E after polyp removal in 2012.Started to have really bad period pains after starting 3 cycles of Clomid and during my routine annual health check, they found "fibroids" on one of my ovaries.Had a dozen second opinions about my condition-ping-ponged everywhere:TCM,fertility specialists(suspected adenomyosis-it is not treatable, implantation failure cos of enlarged uterus, high chances of miscarriages etc)

I just had a laparoscopy done in April.removed 1 tube and endometriotic cyst.confirmed diagnosis:adenomyosis.

Bit the bullet to have lap surgery done despite having a high chance of lowering ovarian reserves.
IVF is my last resort.I just wished I had considered taking the path earlier(hubby was against invasive procedure but finally caved in.Sperm morphology not ideal)
My briefing is scheduled for this Fri but I hope to start my IVF journey in June(time to recover from surgery plus my hubby out of town for work)

Let's all pray hard for good news for everyone here:)

Hi Storkeblessings, nice to meet u here. Hope all the best for ur upcoming ivf... Will be coming soon :) it is gd tt u and hubby decided now rather than later, and 35 is not old at all to try for a kid. Still high chance to do so. Always tell myself, that if Fann Wong can bomb a baby at 42, all of us can... Do u have to do the counselling session at KkH? I find it qt good, u might want to give it a try.

I tried ivf since last yr. First cycle poor response so I aborted it. Second cycle bfn. Third cycle I M/C at 10 weeks. Just did an op to remove the fetus and now on medical leave again. My next cycle prob Jul/Aug.

Hi hi-mei,

Im so sorry about your loss, really hope you are recovering well.
Build up your strength and health yeah?healthy mum=healthy baby
Did you do your ivf in kkh? Is the briefing done together with counselling n bloodtests?
It's my first time with kkh-I thought I might as well make use of the subsidy at govt hospitals for the first attempt and see how things go from there.
I've heard stories about kkivf being not as attentive when it comes to monitoring of the patients cos they have so many(eg cases of OHSS) unlike the private practices.I hope it's just hear-says...
how did you find your experience with them?Personalised/Caring?

Thanks for ur words of comfort. Am slowly recovering.

All the briefings/counseling/blood test is done separately. So u have to come down multiple times.

Yes I agree KKH can be more attentive to patients when it comes to monitoring follicles. Think we need to be checked more frequently. But I guess there are way too many patients and anyway I cannot blame them. It is highly subsidized.

Doctors and nurses are caring. My Doctor is Tan HH and he is very nice. U won't get to see ur own doctor at all during the ivf cycle until ER and ET.

Thanks hi_mei! How were the results of your oocyte retrieval when you had that 2 follices? I had my oocyte retrieval this morning and the nurse said that they retrieved a total of 20 ovums but this is regardless of their sizes and quality. I'm scheduled to see Dr Tan Heng Hao on Thursday at 9.10am and embryo transfer is tentatively scheduled on the same day too. :) Guess I will only know then whether the ovums are fertilised and are good quality embroyos... One step at a time!

I'm so sorry to hear about your recent miscarriage and I'm sorry for your loss. I can empathise as I also miscarried last year. Hugs. Let the body heal and you can try again soon. Take care and keep healthy! Stay strong and know that we are here for you too!

Hello storkeblessings, I agree with hi_mei that you are not that old. Fann Wong and Christopher Lee have also been my "inspiration"! Personally, I feel that some of the nurses are not as friendly in KKIVF as in they do not really smile at you, but they do their jobs well. Dr Tan Heng Hao is really very nice too. I saw him just before I was knocked out for the oocyte retrieval process. I think the staff at KKIVF Centre are just very very busy, but they really know their work. Although I have to wait for hours sometimes, the subsidy makes the wait worthwhile. :)

Just sharing my oocyte retrieval this morning: I was scheduled to be at KKIVF Centre by 7.30am and I arrived on the dot at 7.29am to press Q2. The nurse at the counter then briefed me on the procedure and we were required to sign some forms (sorry, I cannot remember what forms they are now). We also handed over the hubby's semen sample to the nurse. The nurse will ask you when it was produced and also asked for your hubby's contact number so that they can alert him once your procedure is done. You will then be brought to the changing room to put on your gowns and cap. There are lockers for you to place your belongings inside and a toilet for you to empty your bladder.

You will then proceed to wait at an area outside the operating theatre before the anaesthetist comes over to brief you on what he will be doing and also listen to your lungs. A short while late, the nurse will also insert the IV drip needle into your hand before you proceed to the operating theatre. There is a bed with stirrups for you to rest your legs on (which also puts you in a super awkward position, being exposed to everyone there). A short while later, I saw Dr Tan Heng Hao which was very reassuring. An oxygen mask was placed over my nose and mouth and the anaesthetist came over to administer the sedation drugs. Just before I knocked out, I remember feeling the vaginal probe inside already. I almost panicked cos I was worried that the drugs haven't kicked in and I will be feeling the pain. Shortly after I could smell this weird drug and I knocked out. I think this was about 8.30am.

I woke up in the recovery room feeling bad menstrual cramps, and I kept feeling sleepy. Slept and slept until a nurse came over to make me shift onto another hospital bed. She then offered me a cup of Milo. Soon after, I was sent on my way to get changed and made to pee. It was about 11.10am when I saw the nurse and she told me that they had retrieved 20 ovums. I experienced bad menstrual pains and when I peed, there was also fresh blood stains. Apparently, this is normal. I went for lunch and returned home to rest. Slept the whole day away and now I feel much better. Just a few pointers: I felt (and still feel) bad menstrual pains and also had bad cramps when I try to pee/pass motion. It's much better now though, so I hope it will totally go away by tomorrow. Will keep everyone posted after my appointment with Dr Tan on Thursday.

Btw ladies, have you all considered TCM for your hubbies? It is less invasive and might help? I'm not sure it will definitely work though. For me, the problem clearly lies in me but then again, the last sperm analysis that my hubby did was 5 years ago. Will ask Dr Tan if I can have a record of the hubby's sperm analysis this time. :) Hopefully Dr Tan will give me a copy.

Mumwannabe1984, glad ur ER went smoothly. It is a v gd no of eggs collected. How are u feeling today? Cramps should be better now :)

When I had two follicles, they did retrieve more eggs but obviously the rest were useless (way too small), I actually only had two usable ones. The only thing is I am glad those two follicles had usable eggs and they cld become two embryos. But that cycle I miscarried at 10wks. So back to square 1 again.

I don't think KKh does sperm analysis for ur hb ivf semen sample unless it is for iui or specifically for sperm analysis. But u can try to ask la :) wish my hb would go and tiao his body then mb I dun have to do ivf. But u know guys...to admit they got this prob is v lose face to them.

I had 2 ectopic pregnancies earlier. I went to see him yesterday becos I read ectopic via IVF is lesser chance compared to natural birth. However, he told me given my background the risk of another ectopic is the same be it natural or IVF. Sob sob. And he said if I want to be pregnant, I must be mentally prepared for another ectopic if not just forget it. Im lost...

Pregnancy After An Ectopic Pregnancy
If the ectopic pregnancy was treatment without the removal of a tube, a woman still has a reasonable chance for future successful pregnancies.
Removal of a tube may not affect her chances of getting pregnant if the condition of the other unaffected tube is good.
A woman who had an ectopic pregnancy is at a slightly increased risk for another ectopic pregnancy. Hence, she should speak with her gynaecologist about her risk and the measures for early evaluation of her next pregnancy.
A previous ectopic pregnancy. After one ectopic pregnancy, the chance of having another one is about 1 in 10. If you've had two or more ectopic pregnancies, the chance that you'll have another one is at least 1 in 4.

Hi Jkoh,

Poor thing...Im so sorry to hear that...you must be feeling quite down.Big Hug
Did Dr Tan perform laparoscorpy to check your womb to see what might be causing the repeated occurrences?

Jkoh, I am assuming u still have ur tubes even though u had two ectopic? U can do HSG to check why u keep having ectopic, maybe it is some inflammation in the area that causes embryo to implant there.

Yes, it is true iVf cannot prevent ectopic pregnancy. But u can find out why u keep having it and maybe ur issue can be solved some other way other than ivf.... All the best k, don't give up.

Hello hi_mei, KKIVF Centre called my hubby this morning to go down and produce another semen sample cos yesterday's oocyte retrieval only yielded 1 usable ovum, but another 2 matured this morning and hence they needed him to provide a fresh sample to do ICSI again. Did you also do ICSI for your past IVF cycles? I keep reading online about the higher risks of birth defects and am worrying whether I'm doing the right thing to choose the IVF/ICSI route. This is driving me nuts! I guess it is also too early to worry about that now, when I should be worrying whether the eggs are of good quality and can be fertilized. Sigh. Hopefully they become good quality embryos and grow into healthy babies. I don't need them to be ultra intelligent, just normal will do.

Hello jkoh, I'm sorry to hear about your ectopic pregnancies. Maybe you can ask Dr Tan if there is anything that he can do? I agree with hi_mei that a HSG might be useful if you haven't done it yet.

Separately, I'm seeing TCM at Thomson Chinese Medicine and I do find that my general health has greatly improved. She actually wasn't keen for me to do IVF cos the hormone jabs are really harmful to the body and as I had gotten pregnant on my own previously, she couldn't see why I can't now. But since I've decided to proceed, she helps by giving me TCM to support the IVF process and also acupuncture. I must say that her medicine and acupuncture cured me of the night sweats and hot flushes!

Try to think positively mumwannabe1984.
You,your hubby and doc have all done their best.
Now just relax and pray for BFP:)
Sometimes I think ignorance is bliss...esp when I read stuff on the internet, really scared me too
and make me overthink things.
"Test-tube babies have higher rates of birth defects, and doctors have long wondered: Is it because of certain fertility treatments or infertility itself?
Dun worry unduly...distract yourself with your hobby,tv and keep yourself busy until you get your call from kkivf.
Meanwhile, try not to google anything for now(i know its hard cos so emotionally/physically heavily invested) and we all want to know about the outcome of the procedure.
Good luck!

Sigh.. Dr Tan said no one really knows the cause of ectopic. I have one tube removed on the 1st ectopic, on 2nd ectopic the other tube is "saved" but its no longer prefect according to Dr Tan. He asked me to try on letrozole and later if it cant work then IVF. Getting BFP is already hard... imagine BFP follow by 3rd ectopic is omg. Im struggling... dont really know if my heart is strong enough. Hi mei, doc has checked and confirmed I have no inflammation and my other tube is perfect but still 2nd ectopic happened.

Jkoh, nvm, if u trust DrTan just try again with the letrozole. Cannot then ivf. Whether or not ectopic u cannot control... But even with recurrent ectopic, odds are still in ur favour that it would be a normal uterine pregnancy... All the best babe :)

Great that ur eggs managed to mature... Yes I did icsi for all my eggs, but it is out of no choice. My hb sperm is not enough and quality no gd, cannot do normal ivf fertilisation. I think nowadays the standard practice is to icsi. Better than later fertilisation cannot, which there is a risk of it happening. Then u got no embryos. I have confidence u can proceed to ET. Think positive k :)

Hi mei, thanks! You are so comforting! Becos my last ectopic was on 31 mar, im thinking of resting for 6 mths before I try again! You must rest well and eat plenty good food too!

Hello all, sorry for the "disappearance"! I had my embryo transfer on Friday afternoon (9 May 2014) and Dr Tan informed that there were 3 embryos. He transferred 2 embryos and shared that they may freeze the 3rd embryo if it survives. I have not heard from them yet and understand that I will be getting the full report on the embryos and all when I go for my blood test on 23 May 2014 (Friday). The transfer was quite painless and I was allowed to lie down there for about 10 minutes when waiting for the embryologist to come and share the details of the 2 embryos.

The 2 embryos were of grade 4 quality and I could see that 1 was at least 6-8 cells and the other was a 4-cell embryo. But I know that embryo quality does not mean anything... There are too many variables/factors involved : a) whether the embryos will continue to develop when inside the uterus, b) whether they develop into blastocysts, c) whether the blastocysts will hatch, d) whether the blastocysts will successfully implant, e) after implantation whether they will continue to grow into healthy babies and f) successfully deliver healthy and happy babies safely ... Please continue to keep me in your prayers! Time during these next 2 weeks will be crawling by, but I will try to remain positive and keep thinking happy thoughts! But these few days I keep being temperamental and having heated arguments with the hubby. :( I wonder if it is the progesterone pills that's causing it or I'm just being emotional/petty...

hi_mei, jkoh and storkeblessings: Must take good care and have a full recovery before you all embark on the baby-making journey again!

Hi Mumwannabe1984

All the best... Grade 4 is v gd and not easy to get, honestly :)
In the mean time rest well. Don't worry abt heated arguments. If the embryo were to implant, it will even if ur mood is bad. Just telling u in case u were worried... :) it is ur first cycle right? My first cycle I also argued a lot with hubby. Think it is first cycle stress... Subsequent cycles were ok...

Mumwannabe1984, sending u with lotsa luck n blessing! Hope u will see BFP soon! Jia you. Btw, are u on hospital leave, mc, or back to work?

hi_mei, yes yes first cycle. Hopefully it implants thanks so much for the encouragement!

Thanks jkoh, I hope to get the BFP too! :) I'm on hospitalisation leave for 18 days until 23 May 2014, which is my scheduled blood test date.

I had my IVF procedure with Dr Tan HH since Feb this year. We had the transfer of 2 embryo with 5 frozen and the 2 weeks wait is very painful time. So much anxiety inside. Most important thing is to keep positive and happy. Do not overstress and Rest well enough. Unfortunately, when I did my blood test after the 2 weeks, my embryo did not survive. Most of my friends told me it is okay as they are not exactly form yet but to me, they are cause they were the initial process of my baby's future. Hence, I was very devastated as if I really loss a child inside me. Dr. Tan HH was very understanding and gave me a very good advice when I went to see him last week. Then, he told me we will be thawing my embryos and do the transfer again in June. This time will do 2 cycles together if the 1st one fail again without seeing him. He is very nice and thoughtful. For now, I am seeing TCM and making sure whatever I did wrong last time does not happen again. This is to hope my womb is strong enough for my baby to survive. Hopefully, alls well ends well for myself and everyone going through IVF. Be happy.

hi_mei, yes yes first cycle. Hopefully it implants thanks so much for the encouragement!

Thanks jkoh, I hope to get the BFP too! :) I'm on hospitalisation leave for 18 days until 23 May 2014, which is my scheduled blood test date.
Hi sorry I haven't been logging in recently but hope ur BT went well. All the best!!!

Mummypanda, jia you! U will succeed eventually! Positive mind, stay healthy. Your tcm is via chinese medicine?

How are u? Haven't heard from you for a while. Hope you are doing well+

Hello MummyPandaWannaBe,
I'm seeing Dr Tan HH too! Saw him in Feb this year and did my IVF from April 2014 onwards (as you can see from my earlier posts). You have 7 embryos which is considered extremely good! I only had 3 embryos and transferred 2 on 9 May 2014 (the remaining one was frozen). I'm so sorry to hear that the embryo transfer did not work out well. I know that this must have affected you very much but as you say, we must keep positive and happy! Dr Tan is really a very, very nice gynae, I totally agree! June is just around the corner and I hope the embryo transfer this time round will be successful for you! I think it's a wonderful idea to couple IVF treatment with TCM treatment. I did take TCM medicine and many acupuncture sessions before oocyte recover, before embryo transfer and during the 2 week wait. I'm seeing Dr Chen Fang at Thomson Chinese Medicine (inside Thomson Medical Centre) and I find that she has helped me to improve my general health. I also read online that acupuncture session does help with implantation and so, maybe your June embryo transfer will work out perfectly! :) Cheer up and jia you! Remain positive and stay happy~ *hugs*
Mumwannabe84, how are u? Hope to hear positive news from u soon! Good luck!

Hello jkoh, hi_mei,

I'm so, so so sorry for not responding earlier! I had my blood test on 23 May 2014, which was 14 days after the embryo transfer on 9 May 2014. The result is positive! But I'm very, very cautious because of my past experience and while I am trying my best to be as positive and happy, I'm also managing my expectations. :) Just praying very hard that I can get to see my baby's heartbeat when I go for my ultrasound scan on 4 June 2014 (next Wednesday). I'll be exactly 6 weeks then and some websites do say that it might be too early to see the heartbeat at that point in time. But I'm praying really hard that my baby will be healthy and alive and growing well! :) One step at a time! And I wanna say, thank you for all your well wishes and prayers. Please continue to keep me and baby in your prayers... I really hope that this pregnancy will be smooth and baby will be healthy and alive in my arms in 9 months' time...

Let's all pray hard for good news for everyone here:)

storkeblessings, How are you? Are you starting your IVF journey soon? Remember that we are all here rooting for you too!

Congrats mumwannable! Do rest well and eat well!

hi_mei, Are you starting your IVF process in July/August? Take this time to rest your body and take in lots of tonics! Can I also ask whether the gynae had explained to you why you miscarried at 10 weeks? That's like almost the end of the 1st trimester. :(

Congrats gal! Im so happy for you. Rest & eat well!

jkoh, After your consultation with Dr Tan HH, have you decided on your course of action? Hugs!

**********

I'm going for my 1st ultrasound scan at 6 weeks on Wednesday (4 June 2014). And I feel extremely nervous and worried whether I will see my baby's heartbeat. So many milestones ahead! I can only try my best to stay happy and positive and pray hard that my dream of having my baby will come true.
